I can't beilieve I'm saying this but, the ball is in your court. Her actions spoke a lot about her character so you may way to protect your feelings. I agree with you Jack. Usually, the type of people who ask this question are already bashing the girl for being a slut or whatever. QA actually seems to be reasonable and is looking at the issue objectively. It sounds like the girl owned up to having made a mistake and wants a second chance, which isn't unheard of. If the QA is mature enough to accept that, he could try dating her again and seeing where it goes.

It wasn't a mistake. She is back because the other guy dumped her ass. A mistake would be her leaving the other guy because she realized that our friend here is what she wanted. That's not the case. She was perfectly happy being the other guy's fuck toy, and only came back when the other guy showed what he thought of her. A piece of ass that's all.
