Some places like planned parenthood may give it to you without one, but they really shouldn't, and most places I know will not. I know it makes you uncomfortable, but a physician needs to know you as a patient before prescribing a drug you put inside your body once a day. It's how they decide what type and brand of birth control is right for you. It's how they know you are healthy for one, and it's how they know you're not suffering from rare side effects of taking them.
If this is something you simply cannot do, then perhaps condoms are your birth control method of choice.

They have to give you an exam to check for things that might get worse from the hormones in the pill...
Probably an online Canadian pharmacy but you know the type or dosage if never had them before, so maybe try and get a female gynecologist
