Have you met him in person? A lot of American men are sexually repressed, and unless you have already met him in person, there is a chance that is what is going on and you wouldn't realize it. It really isn't that normal for most guys I know to call a woman sexy while talking to her, unless she asks him if she is sexy. He might call you sexy behind your back, but be afraid of you being offended if he said it to your face.
In America, men are told that words like sexy are objectifying to women. Guys get treated like perverts or sexists for simple comments like that quite often. So unfortunately now guys are often too afraid of offending women to tell them the words they often long to hear, like sexy, hot, or beautiful.
Male sexuality really has been demonized in America to the point that a lot of really sweet shy guys are afraid to offend a girl by showing sexual interest. The fact he uses sweetheart is likely chosen on purpose to show his respect for your personality, and not his longing for your body so that he wouldn't offend you.

Maybe it's a generational thing, but I like using sweetheart as a term of endearment. For me it's like, she is very special, touches your heart, etc. Also, I would never call a family member sweetheart. I might say that someone is "a sweetheart", but to use the term as a name implies strong romantic interest for me.
I see other people from the US saying that it's weird though, and I'd never come across that before. LOL

Sweetheart is just a term of endearment. It's usually a thing you would hear out of a guy from the Southeast. It was in my speech right up there with honey, sweetie, sexy, etc. If you don't like it, then tell him.
Sweetheart is a normal pet name in america, yes ma'am. Honey is, but sexy isn't usually a pet name.

"Sweetheart" is usually used by women here in the US, whereas "sweetie" is usually used by both men and women.
