I'm really desperate because this is affecting everything in my life. My relationships, my work, my self image, my health... I've been to therapy but it doesn't really help me.
Is there any hope of getting better with problems continuing this long?
it helps to have a positive self-image. plan out a healthy diet for you- keep a food journal if you have to and plan out your meals for the day, then mark it off once you've eaten it. along with your diet, go to the gym. exercising is not only good for you, but it makes you feel better about yourself.
it also helps to have emotional support- be it from friends or family, so make sure to include some time in your day to goof off or have quality time with them.

You need to stay in therapy, it is a lifelong struggle for some
I'd try to slowly work your way back into just eating normally. The max amount of things you can have is 3 meals, 1 snack being optional if you get the munchies. It's possible to change something even really far down the lane, but it's hard to do it. Don't binge, and don't starve yourself. Both are extremely unhealthy in their own ways. There is hope, but you gotta stick to it yourself. And don't force yourself to vomit either.
