You just don't leave your face alone enough. The cure for acne is simple-- dry out your skin. It's what all acne removal products do, it's the quickest, best solution.
You don't need moisturizers, you don't need soap, detergents or other irritants.. Just something to dry out your skin that won't cause further irritation and probably an anti-bacterial/anti-fungal because if you have a lot of acne odds are you have some exposed wounds.
3% hydrogen peroxide works great. It does all the things needed. Apply it 3x a day.
The rest of your questions don't matter, because inevitably you want that acne gone. Once it's gone, it's easy to maintain, because you'll rarely have reason to scratch/rub acne-free skin, so in turn you'll have less breakouts and you'll know the cure to stop it if it starts.
Try nothing but hydrogen peroxide for 2 weeks, then post back on here. I'm confident you'll be satisfied.

I used to have bad acne back in the day, there's a lot of misleading info online spread by people who don't know what they're talking about. Having consulted with a dermatologist I was told that diet and lifestyle choice can improve/exacerbate acne- depending on good or bad lifestyle/diet obviously. Acne is caused by an excess of sebum, I can't remember if it's glands that produce the sebum (oil) but it's all done under the skin- which means topical treatments are usually ineffective.
I was put on antibiotics by the doctor which didn't work and eventually saw a dermatologist who put me on accutane- which shrinks the sebum glands- after the course of accutane the glands become smaller so less sebum, no oily skin, no acne.

Welcome to the bad gene pool of acne :/ My mom had no acne, and my dad had horrible acne, so I got a mix in between. I've always taken meticulous care of my skin, and when I was on birth control, had it completely cleared up. But now at almost 25, it's come back as mild acne during certain times of the month since I went off my birth control. Unfortunately it can happen at any age and no matter how you care for your skin. The real trick is finding the certain product that works for you.
For me personally, I get either the neutrogena benzoyl peroxide cream, or the clean&clear version (off brands work too). And I put that cream on liberally twice a day. It's the only thing that really works. Face washes and cleaning pads help, but they won't get rid of it completely.
For me, I found that dairy and high-fat foods were the culprit. If I eat too much fat, I usually end up with a breakout lol.

Bacteria. There's a strand of bacteria that causes acne. Get some anti-biotics like
Doxycycline and use benzoyl peroxide treatment. Benzoyl peroxide can really dry up the skin. Other than bacteria, it's also very dependent on your genes. I get bad acne because of my dad's side of the family. It truly sucks, but my face has definitely cleared up since using benzoyl peroxide and doxycycline my face has cleared up.Hormones. My brother suffered with acne before too and he used proactive (one set) and its all cured now.
Its honestly a turn off for me but only when its too much.I read somewhere that sometimes acne is caused by some sort of health issue in your system, you know, it's not the classic bacteria story and such. I would go to a dermatologist or a general doctor, get a blood test etc.
How old are you? It could be your hormone level is off. Have you thought about going to see a dermatologist?
Some people get acne purely based on genetics and not lifestyle. At this point, it's just best to wait out for puberty to be fully over so your hormones balance out.
Go to the doc and get a product called Accutane. It is a miracle drug for acne.
Its just hormones. But go on with living healthy. It certainly helps reducing. Next doctor visit, talk about it.
