This is one of those situations you're allowed to call the police/ambulance and get him to a hospital.
Does he realize it has higher than a 50% mortality rate if left untreated? It's also infectious so he's being a selfish bastard and could cause the death of others. Tell literally everyone that can make a difference...

That's like a totally treatable disease. Is he suicidal or something?
He should! We are required to get tested for this every year where I work. He needs to get treatment.
You need to tell him he is playing with fire and that you want him around a long time and needs treatment.
Does he have issues with hospitals for some reason maybe? I know some people get really freaked out at the idea of having to go into hospital
