I would say ask your doctor about it or maybe even call a local nail salon they could probably handle it for you. My stepmom had an ingrown toenail and they had to take off a lot and part of the nail bed was exposed which hurts, so they made a custom acrylic piece until it had grown back correctly so she was comfortable and nobody knew! Either way I doubt this is something you can handle properly at home you are going to have to swallow your pride and ask a professional for help unfortunately. But think of how good you will feel once it's gone! And it's probably pretty common so no one will be judging you.

See a derma coz it might be worse than you think that's why it has been persistent. It is an infection or yeah fungi so it is important to use proper meds to stop it from growing (they multiply faster than it seems). Maybe your med is not working coz the fungi that you have is resistant to it.
Can't suggest any meds.. probably hygiene only and stay away with nail polish for now. I know it can be embarrassing w/o covering it but it'll heal sooner if left untouched. wear close shoes for now. Hope I've somehow helped you.
Anything with a good dose of ammonium nitrate , it kills all fungus type stuff

I have the same problem, it's been there for over a year and it's gotten so bad that my nail acts like it might actually fall off. I tried some OTC antifungal medicine, but my mom just said it probably won't work on me since it's so severe. I'm gonna have to go to the doctor for an oral medication, which I'm not excited about since I've heard it has unpleasant side effects. :/
