Does constantly listening to music permanently damage your hearing?

Or is this only the case when it comes to really loud music? I'm asking because my ears used to be really sharp when I was younger, to the point where I could hear my moms phone vibrating in her purse from a small distance. I never really used earplugs until like two or three years ago and it's become a daily habit for me to listen to music while doing homework, travelling and well.. anything else basically. And I've started noticing that I sometimes unknowingly put it on it's maximum volume when things in the background are really loud and I'm scared that I'll go deaf at a young age.

I've only had trouble with my ears like two times (one of which was last year and the other one was a couple of days ago while playfighting with my brother) and it was like all noise was sort of in the background. Like I was under water or something. Do you think that I have severly damaged my ears and can I still improve my hearing?
