Well you can't change your genetics. Maybe if you cut back on some of those bad habits, it can slow the aging process. You can also change your hairstyle, makeup, clothing or lose/gain weight and that will help. Those things might drop a few years off your perceived age but if you look 25 its probably not going to help you look as young as 15 so I think your goal is unrealistic.
by the way plastic surgery shouldn't make one look less attractive unless its extreme or botched.
