Is she feeling pain when she eats? Drinks? Any of that?
If so an easy way to bring it up is that the only way it will get fixed is if she goes to the dentist. My girlfriend is the same way and it is just something you just have to keep mentioning.
Don't tell her she looks bad find things that make it sounds it is helpful and heals. An example of that is "Oh jeez my mouth/teeth hurt." respond with something like "Do you have a cavity? You really should see a dentist (insert cute nick name / term). You will feel a lot better. It'll stop hurting."
You may have to pester at it over and over, but if she really cares and values what you say she will eventually just say to herself, "Maybe I should..." or say to you "I don't know..." you can make her feel better by suggesting that you can be there the whole time.
This is a hard situation to get by but I personally have the same thing. Your teeth are with you through life. You take care of your house, right? Your car? Your dog?
Why not yourself?
Be commanding rather than demanding.. Make her an appointment and tell her: "I made you an appointment, I'll be there with you" and follow through on what you tell her. Don't crumble to her saying: "OMFG you didn't talk to me about this first?!".. You guys HAVE talked and she has mentioned her issue with it (fear) and as such all you should do is try and make her issue not so relevant by being there with her. At the end of all this, she will probably be thanking you for actually pushing her to do the right thing.
And NEVER, ever ever ever, do something without talking it over with your spouse first.. You will instigate a HUGE dramatic issue.. Women like to be the "first to know", don't ask me why *eyes cross*

Say I know you don't like the dentist but as soon as you have been you won't have to go again til 6-8 months, say if she wants some support that you will go with her :)
Whats wrong with her teeth - I mean why does it turn you off - does it look/ smell bad?
I mean what if the dentist can't fix it- will you break up with her?
