i sprained my ankles (both of em at different occasions) many times playing basketball. I started doing an exercise in balance for a few years now though, and I haven't sprained an ankle since.
i went snowboarding once and I fell a million times. I fell pretty well though, I would get low to the ground and it wasn't bad. but one time I was trying to stop and fell backwards and hit my left buttcheck really hard. it hurt a lot instantly.. but I was fine.
a couple of times while playing basketball, I would leap toward the basket, get undercut, and land square on my hip. this happened twice, both times hurt like HELL... and one of those times, whenever I would walk and shift weight to the foot/side that I hurt my hip on, my hip would pop... but it seems to be fine now.
when I was in middle school during PE one day we were playing softball. we didn't have gloves though. I tried to catch the ball once and I guess I closed my hands too early because the ball hit my pinky finger and sprained it.
i think that's it... other things were just stingers (like if you got hit in the nose and it hurt for a while but it was fine).

I have broken both bones in my left arm while mountain biking... and then rebroke them the week I got the cast off when I was playing in a soccer game. I broke my right middle and index finger knuckles as well as my nose in a fight once. And finally, I broke a couple of ribs getting thrown from a horse. That last one was my least favorite! XI
I've broken seven ribs total while snowboarding.
I've slipped on a rail and fell, landing on my chest on the rail- that snapped a rib or two. I've hit patches of ice and lost control (when I first started) and smashed into trees, I've been run over... Fun, lots of fun
I also broke my leg playing soccer (both bones).
when I was in cheerleading in high school I landed wrong after doing a stunt and twisted my ankle.

I was playing footie at my university and got caught up in a bad tackle which hit on the outside of my left knee and broke my cruciate ligaments. That set me to rest for half a year.
I've dislocated my knee about 6 times from football and or conditioning, only 3 of them being major and causing a visit to the doctor.
i had to get stitches above my eye playing football once, I've had other minor injuries as well.
If you consider being a couch potato an activity , then have been hurt doing that. My legs get partially paralyzed each time I become a couch potato.
