Tell your doctor at your next checkup. If you don't have one for a while, go to your school to the coach/office/sports department and ask about the physical forms for participating in spring sports. Tell your mom you need a physical.
Alternately, pretend you're getting bad headaches or some random ailment that can't be medically confirmed and ask to go to the doctor. You've got excellent chances of her believing you're actually sick if you do this on a Saturday and not a Monday.

ok if your mum doesn't believe you when you think this is serious... then you have to seek other adult advice.. or even better if you can. .speak directly with your doctor.. and ask to get checked up.. don't just let it pass by because your mum won't listen to you.. that's her problem.. but you shouldnt have to even possibly suffer because of it.
Is there a school psychologist/social worker at your school? Even the nurse should be able to help you. Schools usually offer several ways for students to get help without going through parents first.

You probably already know more about your disorder than most people do, but it is psychological in origin and can be treated with appropriate counseling and sometimes with appropriate medication.
Moskau has some good advice. Find a way, any way, to get in and see a psychologist, psychiatrist, physician, or other individual sufficiently trained in counselling and/or health care that will keep your confidentiality. They may be able to help you break it to your parents as well.
