Acne can also be very diet related. I cut out oils from my diet and started working out and my skin cleared up completely. But yes it is also very hormone related. Its not caused from testosterone but an imbalance, usually during puberty and unfortunately later in life, its considered "unattractive" in men because it could mean you are still evening out hormonally and therefore not fully grown or just have a slight imbalance in testosterone which can actually affect your sperm count and therefore reproductive viability. Men and women alike both need a balance of estrogen and testosterone to function perfectly and often imbalances or variation can cause eruptions on the skin and sensitivity to the bacteria on the skin.
The short answer is evolution. Acne is not just a sign of testosterone, and even men with lower levels of testosterone get it. However, it can be a sign of poor health, eating poorly, and low hygiene. Women would obviously prefer a healthy mate, and as acne is seen as a sign of poor health, it would of course be unattractive. So, it's simply a matter of instinct, as we are biologically attracted to mates that appear healthier and therefore more likely to bear healthier offspring.
Acne isn't caused by just testosterone. It could be bad hygiene or poor diet. But either way, excess testosterone resulting in acne= bad genes.
Hmm I've never had acne but that doesn't mean I don't have testosterone. I've always had problems with dry skin even during my teen years so does that mean I'm less manly than you? Rubbish.
I spend at least 15-30 minutes on skincare each day to keep it moisturized. I'm sure there's something you can do to prevent acne as well.

The things you have listed are merely some factors that may aggravate the issue, but not they are not the root of it. For example, have you seen a baby with severe acne? In Africa, they have millions of babies that have no access to water therefore they cannot wash their faces. However, none of them suffer from pimples. Hormones and genetics play the biggest role in acne formation.
So why are they unattractive?
I get that sometimes people still suffer from acne regardless how clean they are. It's a crappy hand that life dealt them, and it sucks. I just told you why acne is unattractive: because people associate it with poor lifestyle habits and skin care. People always judge books by their covers. I don't find it unattractive, I don't mind it. My boyfriend has kind of bad acne, I just ignore it.

A Dermatologist told me, acne is a problem not caused by diets, nutrition or lack of water. Its just heredity. Also confirmed by a nutritionist. But I can even say acne is unattractive because it is.
