well first off gifts shouldn't really be given with the idea of expecting something back in return. for him, I hope its not otherwise but I'd expect he knows this too. I think only thing you can do is just be thankful, appreciative, and happy that you have someone who cares for you, which I'm sure you are all these! and its not just by spending a lot of money but the custom making that was thoughtful of him, so don't worry or feel pressured, thank him in your own way; understandingly if you wish for him to return it you can always let him know, just make sure you communicate to him what you're thinking and feeling!
Don't feel bad, he isn't giving you an expensive nice gift like that for the purpose of you giving him one back of equal value :P

Accept the gift wholeheartedly and forget about the rest. Most guys who would buy a lavish gift for their lady would never expect her to do the same for him. Guys don't look to their ladies to equal them in income or gift giving. They typically buy expensive gifts as a masculine way of displaying their affection for you. It's like taking you out for dinner or buying the really nice engagement ring. It's a guy thing.
Further, he must be well aware about your situation. So, instead, give him thoughtful gifts that reflect your love for him, but don't cost much money.

just accept it. He wanted to get it for you to show how much you mean to him, if you don't accept it, that will send the wrong message
