I'm just wondering why it seems like only in western countries is it neccessary to have extra requirements vs in poorer countries.
For example it is more likely to result in death in western countries if there isn't a qualified doctor/nurse and someone to provide stitches to the area later. Whereas child birth isn't supposed to be something that results in death and indeed many non western countries manage it without.
Is there something I'm missing?

Yeah you're missing the child mortality rate in poor countries.
Also the us is a level beyond the rest due to fears of lawsuits.1
