Pretty much how you asked; you get a good stream of water, open your legs, and make sure the stream of water hits the clitoris. The key is that it hits the clitoris, because that's where the majority of the nerve endings are.

If it's a bath, the girl would do what I said above - open legs, aim water at clit.
If it's a shower head (has to be the detachable kind), she'd detach the shower head, open her legs a bit, point the shower head towards her vagina, and aim it at her clitoris.

I haven't done this type of masturbation in a long time, but personally, I find the detachable shower head easier to use (compared to just lying in a bath and having the water stream down) and much more stimulating, because I can kind of swirl or move the shower head around so that it hits the clit a little differently each time... it's an added bonus if you can change the different types of shower head streams (jet stream, pulsating stream, etc.)

I don't use the showerheas personally - but understanding that the clitoris is our hotspot, and is on the outside and more frontal than between our legs, would be key to understanding, I think.
