It is never okay to insult someone's physical appearance that they can't control. This isn't a gender issue. Men insult other men for having small penisis and women insult other women for having to small or too big of boob's, chicken thighs, cellulite, bad hair... The list goes on and on. People in general need to grow up and not care so much about looks
I know that men are pretty sensitive about their penis. If I've actually seen his penis I absolutely will not joke about it because I don't want to hurt his feelings. If I've never seen it though, then I think those jokes are fair game since he knows I don't know what it looks like.
