It can be hard to impossible. If the orgasm is close, instinct drives you to thrust faster and deeper and harder into her vulva. Pulling out as a birth control method is remarkably poor - even if the guy has the self control and presence of mind to get out in time. There is usually an amount of pre-cum already deposited and that can have sperm in it.
