A guy can but it's very rare and apparently very difficult to do, I've only had it once or twice... Thing about it is though, for me I definitely had an orgasm but it was less pleasurable than when I do ejaculate. It is possible to hold back ejaculation when orgasming by clamping the PC muscles really tightly (this usually requires doing Kegels for quite some time). Some guys can achieve multiple orgasms that way. I've only been able to pull it off once, it takes considerable practice I think.
