Normally, I have no trouble getting a girl plenty wet via foreplay, but twice I've had girlfriends who had to take medications that had a side effect of vaginal dryness, and artificial lube was needed. In both cases, the girls were really self-conscious about it at first, but we both knew it was the meds, and in both cases, once she was done with them, things went back to normal: full waterfall!
But even without meds, a few women simply don't get wet enough to have sex comfortably, and a few more have trouble STAYING wet through a medium-to-long sex session (i. e., anything over about 10 minutes), no matter how wet they were at first. That's mostly just a genetic issue, but I'm sure it can be a mental/emotional issue in some cases (i. e., an abusive past, etc.).

It depends on the girl, but its perfectly normal either way we are all different, personally i almost always use lube before sex because its more pleasureable for me at the time and i am a lot less likely to be sore afterwards
