Not on a regular basis, but under certain circumstances, yes:
1. She's not fully ready, or she's very tight. In either case, it can cause the skin to stretch from the head towards the body and can be pretty painful. Quick fix: lots of foreplay or lots of lube. I usually insert 'just the tip' and pull out, insert a bit more, back out and so on until I can get it all of the way in. This helps slowly stetch her out and naturally lube everything up, even when she's super wet after foreplay.
2. Wrong size/too small condoms. I used to get so hard sometimes that it would hurt; it was because the condoms were too tight and cutting off circulation and turning my penis dark red/purple. I was using the wrong size condoms for the first 8 years of my sexually active life. One girlfriend finally brought up that I was taking awhile to put on condoms, and that my penis looked funny in them. She convinced me to let her measure and after we did some research, we found out that I should've been using large size condoms. Tip: Girth is more important than length when it comes to condoms. A regular Trojan will unroll up to 7.9" long; you just unroll as much as you need. Girth wis where condom sizes really change. We found a quick test suggested for girth. Put your erect penis into a toilet paper roll; if it fits comfortably, use regular condoms; if it's uncomfortably toght or doesn't fit, buy large.
I put yes though it very seldom ever happens, and when it does it doesn't feel really bad.
I have had a few encounters where due to duration and frequency the glands on the head of my penis were rubbed raw and was put out of commission until it healed up... And there have been many other times where my penis is in just a bit sore around its neck and base, but that kind of pain usually feels pretty good ;)

Not generally, no. It is possible that if foreplay went on for a very long time, that we'll feel a dull ache in our balls (blue balls) but that gets relieved very quickly once we orgasm. Also, depending on the positions and the vigorousness of the action, it's possible for our balls to get hit in uncomfortable ways.
