It means they used to be a regular man or woman, but realized they were really supposed to be an entertainer for a noble's court, where they now juggle, sing and play music, and tell jokes, sometimes even joking about their employer where no other citizen would dare. They can't necessarily spell well, apparently, but I suppose that would be expected of a member of a feudal society.

A sex change is, in my opinion, basically just a "costume". A trans is the same person and gender they where born as with no more than surgical "make up".
