If you smoke or if you've just drank alcohol before (especially beer) you should definitely brush your teeth since these things give people a very bad, stinky breath. Also, make sure to brush your tongue as well. All the bacteria that produce the bad smell sit on your tongue, not on your teeth. So by brushing your tongue after brushing your teeth, you can make sure that you'll really be minty.
As for my own life, I like the approach of @fauchelevent. My girlfriend and me are pretty similar. We've been together for a long time and to be honest with you... being in a long relationship can make you really lazy sometimes. So sometimes we brush our teeth before sex but sometimes we don't. If we have sex before going to sleep, we just naturally brush our teeth but if we have morning or afternoon sex, we're just like "oh screw it" ;-). I also have to agree with @fauchelevent that it can sometimes feel a little awkward if you're all horny and heated up already and the other person suddenly says "please wait here five minutes while I go and brush my teeth". It ruins the atmosphere a bit. But then again, my girlfriend and I don't smoke and don't drink alcohol. If you smoke, you should definitely brush them.

Breaths don't smell bad after smoking weed but you should always brush your teeth before seeing a girl or before going out in general.
Brush, or mouthwash, or gum/candy, or one of those breath freshener strips (they fit in your back pocket, dissolve quickly and work really well) are all good ideas. Bad breath is a huuuuuuge turn off.

Yes, but brush an hour or 2 beforehand. Never brush right before sex and never right after. Use mouthwash if necessary.
Shower, brush your teeth and use mouth wash before being intimate. I want my mate to be digging the total experience, not tolerating certain things. Use 100% total hygiene and she is more likely to go exploring and doing her kinky stuff!
