It takes 1 finger and 1 thumb - Squeeze claps together and snap your fingers. It's very easy. I've had a lot of practice. In a few relationships I was the "Hey could you unsnap by bra?" guy.
Have you ever seen a women put on a bra? 99.9% of the time the put it on backwards, snap in the front. Clamp the bra, rotate the bra round their midsection 180 degrees, pull it up to their breasts and pull up the shoulder straps. They mostly do the same to remove it.
Removing the bra while she keeps a shirt on, is slightly trickier but requires arms to come out of her sleeves one at a time to remove the straps.

yes it is very possible, my girl friend actually just left. we have been boyfriend/girlfriend for about 3 weeks and while we were making out if you go up the side, move to the front the just pop ur wrist up and it should slip over. all this done with the bra still strapped.
you should easily be able to slip your hand underneath her bra unless its too tight for her, you would then sort of have you hand on her boob with the cup of the bra on top of your hand..
if she's wearing a low cut top you would probably be able to go down from the top of the bra..
you could move the bra up so its above her boobs on her chest just below her neck and then youd have better access to her boobs..
however, if you did want to take her bra off, an easy tip is the take the clasp of the bra between your thumb and first finger and it should literally just come undone instantly :)xx

It definitely increases. They also swell & get bigger.
very possible. it might not be very graceful but it's possible. some girls wear their bras too tight and that won't give you much room to move your hand in.

it is possible...it just might be a little uncomfortable for the girl, and it also would not be that hard to take the bra off itself, if you do that it would show her you want more, and it would be more pleasurable for both of you.
