the size of a womans vagina only changes during arousal and changes to accomidate whatever is entering the vagina. it expands in both width and depth. after arousal is over, it goes back to its original size. kegel exersizes work out the muscle that makes up the vagina so doing them will make the vagina muscle larger, just like working out any other muscle. the increase in muscle volume is what translates into a tighter feeling for the man. the vagina does not stay streched to a different size after having mulitple encounters with a larger penis. Women have different sized vaginas just like men have different sized penis'. A small vagina will strech more to accomidate a larger penis/dildo than a larger one will but it is all relative to the size. the only thing that would make a vagina feel 'loose' would be a loss of muscle volume, which would be uncommon unless the woman was in poor health or had a disease that caused her muscles to atrophy.

Tampon size is due to how light or heavy the menstrual flow is (small=light flow, medium=medium flow, large=heavy flow, extra large=deluge). It has nothing at all to do with vaginal size.

I'm not going to get into an argument with tampon wearers - if a girl is on a large size and yet has a light flow that is unlikely to make the smallest tampon a perfect fit. It's a combination. The vagina is a muscle, just like any other in your body. It was built to expand and contract accordingly for whichever penis size you're currently using.

While you are having sex with him, if you try having sex with a smaller guy, you will probably notice the difference.
