I say my bf's name..which is unusual for me. Id love it if he said my name, that would probably give me such an ultimate O that I couldn't walk for a week...but he calls me baby. Most likely for the reason hedoniste gave. All it takes is one slip up...ever..and a guy will never want to make that mistake again...and he's accidentally called me by his ex's pet name (thank goodness I love her, or it would piss me off...) so I don't hold it against him...but yeah, if he said my name too that would put me over the edge.
I usually just babble whatever is at the tip of my tongue. Which is yes, usually his name. Or "Love." Or "Fuck."
I like her to say MY name. Not too often or over the top. This way I know that she is with me and not thinking of someone else.
Love the emphasis on "MY" name. I just stick with "baby". Hard to keep track sometimes, so it saves me some tense conversations down the road.
