Neither. Blue balls is an engorged prostate and muscle strain at the testicles. During arousal the testicles lift to load the seminal vesicles and prostate swelling them and stretching the local muscles. The longer we are aroused, the more strain and pressure there is. The only time a guy will have blue balls is when he is in the middle of a sexual act and or sex and only goes halfway and doesn't ejaculate.
