It's not a good idea to be this determined to worry about something, nor is it a good idea to put too much stake in orgasms. They're iffy, fleeting things, and often beyond anyone's control.

I'm tempted to withhold some of the possilble reasons he couldn't come (second orgasm is always more difficult, discomfort, fatigue, distraction). Nothing would be less helpful then treating his orgasm like a science project, and trying to tweak the variables.

What should you do? DON'T WORRY ABOUT IT. I know you're struggling against it, but it's really the best answer, and the sooner you accept it, the better.

ok, I have to tell you. This is not an uncommon situation. Most men do not talk about it. I have had gf's who get upset when I do not cum. It's definitely not something you did, or where you weren't as good, or because it was too cramped in there.
