rings are better than barbells, unless you are using horseshoes or curved; make sure jewelry is not too tight (if the piercing is dark at one, or both, ends, your jewelry is probably too tight) and also check that there is no flaw in your jewelry (nicks, scratches, etc.) as that can trap bacteria.
Also check to make sure this is not an allergic reaction--your skin will usually try to pull away from the metal if it's an allergy. If you suspect an allergy, change to titanium or niobium jewelry.
There's always the risk of infection when piercing any body part, so make sure you continually clean the area with mild antiseptic until it's thoroughly healed (when it's no longer red, sore, swollen, or tender). The skin could become irritated from clothing or underwear rubbing against it every time you move, and it can even tear if the jewelry gets caught on fabric. If you're dead set on body jewelry, choose a piece made of a nontoxic substance (like surgical steel or gold) that lies close to your skin (nothing dangly) and be extra careful when you towel yourself dry after bathing.There is some prepiercing info you should know about, too. And don't forget about the "ouch factor." You have to consider your pain threshold. People who've been pierced say it feels similar to getting a shot. But, everyone's genital sensitivity can vary, and this nerve-packed area can be pretty tender. So, if you feel faint at the sight of needles or don't deal well with discomfort, it's probably not a good idea.

I have no idea about the health related things but my sister has her clit pierced and it's the barbell type and she never complained about it hurting her or anything.

I was pierced with rings but switched to barbells on my nipples. They're fine you just have to clean them well.
I love them both, if not overdone (too many especially down below). Very hot and sexy and such a turn on. Pierced nipples are my absolute favorite.
I just find them very unappealing, major turn off that is.
