Well, you have to tell her, lol. That way the doctor knows what to do ahead of time. Saying anything other than the direct truth can mislead your doctor into thinking you need something other than what you actually need.
Don't be ashamed. Those who work in the medical field have heard everything. Yeast infections are very common.
I suppose, if you really don't want to say it, you can say something like "unpleasant side effects from antibiotics" but she'll probably ask you to be more specific.

just say "a follow-up from my last visit"; if your doc is the one who put you on the meds, s/he will know.

I think you can say that you need to see an OBGYN, for things related to that doctor.
Just say you think you have an infection. Just make it general
Tell her the truth. Women get these all the time. It is no biggie. I've told a female receptionist my balls hurt before (enlarged prostate) no big deal... it's what they do.
