Life does not have to be full of regrets but unfortunately it is.
People don't think that the present day might be their last one where they are healthy and they regret not having been more careful or that their loved one (s) passed away without being able to tell them how much they loved them.
There are millions of examples why each day is filled with small regrets. I regret having eaten this piece of chocolate because now, I will have to work harder to get rid of all the calories.
You may not find this deal on the internet that was available today and tomorrow, you will regret not having bought it yesterday.
The biggest regrets though are the ones that leave you permanent negative impressions such as those that are affecting your deep emotions for your family and loved ones.

Not necessarily. No one can make the optimal decision in every situation. The worst you can do is not make a decision. That leads to the biggest regrets.
Not always but i have regretted in my life and I know it feels like shit. But you cannot change the past instead of regretting just try to make it up for future and do not repeat the same mistake.
