I'm pretty sure its rare that they'd do it. Lets be realistic, the viewers have favourites and in two and a half men its quite evident that sheen was everyone's favourite. He was removed due to some controversy or drug addiction i believe. Otherwise, it was such a hit show, doesn't make sense why they'd change the script and all of a sudden the main character dies. I'm guessing their disciplinary image is key to having the show run successfully otherwise they come into the limelight for the wrong reasons and that may adversely impact the ratings.

Just to create a lasting and unforgettable impression on the viewers ig. Or if the contract has ended for that actor in that particular movie/series.

Contract/salary disputes.
Unreliable talent (in the case of Sheen - drug and alcohol addictions.)
Shock value; trying to keep the audience on their toes and guessing.
Bring fresh new blood in, new characters, keeps the show fresh, they think. (Could have the opposite effect, and lose viewership, as the best, most beloved characters die.)

They aren't favourite characters , though. They might be your favourites, but the numbers don't lie They just no longer pull in the crowds required to make another season.
Also, Sheen was a dick to work with, according to all the people who worked with him on set. Perhaps you're the obnoxious type in your group too.

What would you do if you top actor gets arrested... drunk, on drugs, with a hooker in broad daylight
A lot actors did loose their job due to drug abuse
And especially charlie sheen After he was dry for a year with the AA... first thing he did on the day after was getting arrested with a hooker, so full, the police needed to lift him into the police car... regardless to say that he sat on a cars wheel at the moment

sometimes it's just the actor quitting the job. but sometimes it's a story telling instrument. like if you kill a meaningless side character, you don't care much right. so you need to build up a character first in order to get the maximum impact for killing that character. that's basically how game of thrones works xD

Depends.
Either the actor or actross died in real life or didn't want to continue the show, so the director decides to remove the character from the show.
Or the story actually requires a character to die, usually to create tension or drive the narrative forward.
And sometime the actor or actress did something which the production company didn't like to be conected with them and their show. In Charlie Sheens case his year long drug addiction.

He got kicked because of his drug addiction and behavior. Also lets be honest John Cryer is much better actor than Sheen. Sheen does have his own comedic charisma that makes him likable, but he's not that good at acting in my opinion.

charlie harper got written out of 2 and half men because charlie sheen went on a drug crazed months long bender. he got fired for being an asshole to his boss.

It's typically done when an actor leaves a show, be it by choice or not.

Various reasons, contract, inside differences, new opportunities, or they just want a good ending.

That happened because of drama behind the scenes. Lorre and Sheen hated each other. And because of it, the show got fucked.
He has been sacked so they should improvize with his character.

I think it's that some people are so hard to work with or they demands such an unreasonable amount of money that it's cheaper to kill them off than to continue trying to deal with them.
