1. Don't yell in front of your kids
Yelling in front of your children can scare them into thinking that when they grow up they won't find happiness because you didn't. Not trying to sound rude but just talk calmly in front of your kids. Even small things like parents fighting can get stuck in a child mind for a long time.
2. Watching tv while the kids are studying
If your child is working on homework or studying the TV should be off. I personally listen to music to help me work. That might work with your kids. A TV will distract a child from homework and their studies.
3. Smoking in front of the kids
That can cause lung cancer not only for you but for your child. Not saying you have to stop smoking if you do. Just don't do it in front of your children.
4. Stop Hovering!
Children and teenagers like to have some freedom. Standing over a child doesn't help him/her move faster it just slows him/her down. No offense I know you mean well. My mom hovers and it gets annoying and hard to focus on homework with her staring at me. My advice to you just give your kids some space.
5. Stop teaching failure is ok! Its NOT!
Failure is not ok. Teaching your child this is not a good idea. They will think it is ok not to do their homework and they will fail classes just, because you thought teaching them failure was ok. I thought failure was ok until I learned the hard way. It isn't ok so stop teaching your kids it is.
6. Stop blaming everyone for your child's mistakes
If your kid doesn't own up to what he/she has done they won't learn. Children need to be taught how to be responsible. The first step in doing that is to teach them how to own up for what they have done. Quit blaming others and be the responsible parent.
