Is it okay to ask a girl her name again in case you forgot the first time?

If I'm meeting a guy at the bar, then I appreciate his "memorizing" my name like that, and I do the same with his name: repeat it back to him every so often to check in that I'm remembering right. It's flattering if he likes me enough to try to remember my name for future reference.

Well I guess it depends on how long you two have known each other. If it is your girlfriend, and have been for 5 years. I think she would take offense, a new person in your life, I don't think she would mind. Obviously it differs between people. ^^

You could give her your phone and ask her to give you her number if you haven't done so yet. There's quite the chance she'll add her name herself. In that case, you don't have to tell her you forgot, she probably won't even notice.

Keep yourself busy with something (pretend to search for something in your pocket, for example) so you can ask her to complete the contact information herself if she didn't and it doesn't look too weird.

Some girls might get offended by that but for me in case that is his first time then no.If he forgot again for the second time seems like I'm going to have problem with him.
