Do you have any tips on dealing with lack of concentration/attention span?

I don't know what it is, but for a long time I have experienced problems with concentration. Sometimes I don't, but other times I am incapable of completing a task I sat down to do because I get distracted so easily and a lot of ideas and random thoughts occur, and it's hard to concentrate on the task at hand in general.
