If you have extreme anxiety, how do you work through it?

If you have extreme anxiety, how do you work through it?
I have pretty bad anxiety over the most trivial things and huge things alike. Sometimes it feels as if I physically can't breath and there's a brick in my stomach when I get into one of those moods. Anyone have tips or mind sharing some wisdom with me?
