"Your breath stinks."
Pretty straight forward. That's what SOs are for. They tell you when your breath stinks, when you have body odor, when your clothes don't match, when you are too drunk to drive, when you are being stupid, and when you have a booger in your nose.
It's important to know those things, and if your SO can't tell you, then who can? You can try to be diplomatic, but if that doesn't work just blurt it out.
Stick one of those Listerine sheets in their mouth
In all seriousness, some people really need to be flossing daily. That's where the residual smell lives between brushing, between your teeth or worse stuck to your teeth in plaque that only a dental hygienist can really get rid of. People during this pandemic are seeing the dentist less unfortunately, but it's totally worth keeping your mouth health.

I have been with the same man almost 30 years and with that comes honesty. He knows I am very blunt and he knows it's for his own benefit. He is not a regular devotee to flossing so I will tell him his breath smells like rotted animal flesh.
I find a powerful water flosser is good for people that don't do it manually often. You can add things to the water that benefit the dental health too, or use reasonably hot water.

Hand sanitizer for under your armpits if you don't shave it will get rid of a smell under a minute better than that stupid stick. water floss every single day after you eat. Male and female use of a European bidet.
