How to effectively communicate with an emotionally unavailable girlfriend?

Girlfriend has childhood family issues which are the likely cause of her emotional unavailability. However, she's acknowledged it before and seems willing to work on it. However, recently she appears to be withdrawing, even though things have been going well recently. I would like to have an open conversation with her about the situation but I understand that doing so may make her uncomfortable and draw her away even more. What is the best way to communicate with her so that I make my feelings known without making the situation worse? Some concrete examples (including things I can say) would be helpful. Thanks
