If his name is not on the birth certificate. No. But it would be the most respectful, kind, and loving thing he could do for her and him. It's sad people make these horrendous mistakes and children are born into this world with little to no support. The only legal way to make sure he doesn't is as somebody already says, paternity, and by the family court with the proof of wrong paternity.
Technically, no. I don't think it's mandatory, especially if this was a knowingly unfaithful act and not some weird cuckolding situation. However, the man can choose to, if he feels he wants to. I wouldn't judge anyone either way.

No.. I am pretty sure one can ask for a DNA test to show they are not related and then there are no legal obligations.
