When you are older, and in a relationship for a while, your life often gets busy and stressful and sex can become routine. For those people, "birthday sex" tends to mean that their partner will take extra time, and make an extra effort (and often do things that they skip on a normal day, such as oral or extended foreplay and teasing) and have the kind of wild sex that they USED to have at the beginning of the relationship.

Often, women will "go slutty" for a guy's birthday, perhaps being the one day a year he gets a BJ or anal or gives her a facial or has her dress up or whatever.

It's less relevant when you are younger and/or haven't been in a relationship very long, and tend to do those kinds of things often.
