"Blue Balls" is an expression that kind of conveys how things FEEL, but it is far from medically accurate.
When a man gets aroused, his prostate gland, which both produces and stores semen (not sperm - sperm comes from the testicles and gets injected into the semen stream during ejaculation) kicks into high gear in anticipation of an orgasm and ejaculation. The increase in semen production creates pressure in the prostate gland (which is near the testicles), and that pressure creates a bit of pain, but also some sexual tension that is useful for getting a guy to orgasm.
But if the guy gets highly aroused and stays aroused for a period of time, but isn't able to orgasm, that semen pressure in his prostate build up to very painful levels, like an over-filled balloon, and once the arousal endorphins (which mask pain) wear off, that pain is very, very real. His prostate becomes extremely tender and painful until the pressure is relieved.
An orgasm can relieve the pressure, obviously, but without one, the body will eventually release and dispose of the excess semen. But it can take hours, even days in some cases for this to happen, and until then, the guy can be in pain and tender.
Now, many guys use this REAL condition to pressure girls into having sex (or give BJs, or whatever), and while that WOULD solve the immediate problem, it's not the right answer. The right answer is to not arouse the guy in the first place if you aren't prepared to get him to orgasm. To do so is cruel and mean, honestly.
