Actually, I can fit my whole arm into a rectum nearly up to the elbow, and even carefully curl my fingers to touch the inside of the sigmoid colon, so it's not a matter of too deep.
It is more a matter of being too rough, causing pain by sliding too hard against the delicate tissue of the rectal walls, and most specifically, jamming painfully against the second sphincter which isn't deep at all, but just inside the anal sphincter you are all familiar with.
