No, not the first time, but it has a time or two. In my case my guess is it was caused by too much friction or being hard too long? Maybe he was too rough with it and/or did it more than once? That would cause it to swell up likely.

No it's not normal for a penis to swell up after masturbation. That's very abnormal behaviour and I worry as to what he has done to cause swelling.
