I wouldn't be bothered if me and my SO truly trust blindly each other, and if I know that I am in full control in the relationship (in case the other person is a girl, or in case it's a guy and my SO is bisexual).
And also, I've to be also attracted to that third person first of all, that's really not granted (what are the chances that both me and my SO are attracted to the same person? Meh, not many).
Another requirement is that they must not start dating each other or get in confidence etc. I wouldn't get too personal with them either as well, to respect my SO too in the same way.

Still, you can see it's a very delicate thing and you can see there are many variables that could make it impossible from the start.

When you ask about a threesome, do you mean with 2 men or 2 women. It makes more sense to have 2 men and 1 woman because women are far more capable of multiple orgasms and because a fantasy that many lovers have discussed with me is having a cock filling each of their holes at the same time and getting filled/covered with cum. depends on our relationship. Like, if we agreed to an open relationship anyway, considering my inclination for sexual thrill seeking, I suppose it's kind of inevitable.
