It think its because of parent's/school's lack of teaching those things. You have to wash under your foreskin, just like you gotta wash every other part of your body.
In my country importance of washing under your foreskin is teached in sex ed classes, and they say it should be once a day.

To my understanding, if it is for real really tight, then its better to get circumcision for hygienic reasons. But if you can pull it back, then there is no reason really for that. From what I've heard, forskin can be tight first, but stretching and some "topical steroids" apparently helps. Google is your friend here
