Start? I guess 9? May have been sooner, I used to go shirtless at home and not care, but around 7-8 age, I felt it was weird to keep doing it. Stop? Uh... 16 maybe? Boobs are weird, plus they'll grow if you gain weight etc so it's hard to keep track of.
With bra in my opinion, then again, I've seen them and their imperfections for so many years, so it's probably why I think that way
Braless in public? Never. The last two years I sleep in tops at least, so I'm usually never without anything except when I shower/dry
Maybe a little bigger, but then again having big boobs usually gives back problems so...
Probably daily? It's hard to keep track on, I just touch them sometimes without thinking
