I first started getting therapeutic massages when I was in my first job out of law school. I was carrying a lot of stress and tension in my neck, shoulders and back. The gym where I had a membership had massage rooms where Licensed Massage Therapists could freelance in the evenings.
So I scheduled an appointment with this woman LMT because the only ones on the list were female. I didn't really think anything about that. Anyway, we meet in the massage room and I tell her what's bothering ms. She says that she will leave the room for a few minutes while I "undress to my level of comfort" and lie down on the table under the drape. Since I didn't want to be prudish and since I considered her a health professional, I undressed completely.
About 20 minutes into the massage (while I was on my back) my penis became fully erect. I didn't have any creepy intentions toward the LMT and I was very embarrassed. My penis is average size but the erection was very noticeable under the thin drape cloth. I apologized to the LMT and offered to stop the massage and pay her for the full hour. But she was very professional and not offended in any way and explained that my erection was a normal response to the activation of the parasympathetic nervous system during the massage. She said is was not an issue for her and she completed the massage. And the massage really helped with my stress and I kept going back for her to work on me. Sometimes I would get an erection and sometimes I wouldn't. But it just wasn't anything I worried about anymore because of how professional and understanding she was. I wouldn't have felt that comfortable with a male LMT.

This is not an isolated incident. And you both acted the way you should act in the circumstances. "Undress to your level of comfort" is a very common phrasing as the MT leaves the room for you to prep. A good MT can work with any level of dress or undress, but it can effect the outcome of the massage. If I am doing heavy work, I'd like the client either unclothed or wearing skin-tight clothing. If I'm doing relaxation or erotic work, of course they should be unclothed for the smoothest strokes without clothing or drape interference. People ask if they have to have a drape (sheet, towel) over them. I tell them that the law in our state says that I have to "provide" draping for "warmth and modesty". It does NOT say the client must USE it, though.

It's a fairly even split for my clientele, and usually depends on the type of massage they get. One guy I work on will come in his jeans and a tee-shirt. No real problem since his is not relaxing work, but a lot of stretches and deep stuff. No oil or relaxation for him. A lady I worked with for about 13 years until her and hubby retired and didn't have the money to continue. always wore her panties. Again, no problem. I DID have to do deep work on her in her inguinal crease (groin), so I requested that when I did that work, that she wear ski pants/leggings. Sometimes women will keep their bra on. I will always ask to to unhook it before doing long smoothing back strokes and no one has refused me in over 25 years! And of course ladies who want erotic work go commando! I always undress completely. Clothing is just a hindrance in most cases. But, on the other hand, if I have to take an extra five minutes to keep shifting a sheet around, then that's about five minutes of rest I get from actual "massage". :- )

Re Breast Massage. Of course - No problem there. Breasts aren't even considered "erotic" work. 12-13% of the female population will get some sort of breast cancer- 1 out of 8! They will HAVE to receive breast work. For the other 7, it is helpful to act as a preventative action. Cancer cells have a tendency to establish themselves in "static areas" of the body- areas that don't really move on their own. So, there is rarely initial cancer of the leg or arm, but there is initial cancer of the breasts, lymph nodes, testes, uterus, pancreas, etc. But I digress - - - I have additional certification in "Oncological and Breast Massage" from Cheryl Chapman of New Jersey. When I'm working a female, after working her arms and legs when she's face-up, I'll just ask if she wanted any breast and/or ab work, and she'll just yay it or nay it. Erotic work is different. The genitalia are involved in that, but the breasts are usually also worked "in tandem" with the vagina/clitoris/perineal area since there is also a considerable number of nerve endings in the nipples, in both guys and gals. Re: Forget about the draping. A few times. Once when I worked in 5* spa, one lady just kept walking to the table, flung her robe up on the hook on the wall (nailed it, too), plopped herself down and said, "OK- let's get going, honey". Of course I had to report her actions/comments to my boss. In private practice, there was a female Asian optometrist from California visiting her sister. They both came in and when it was her turn she just laid atop all the sheets, not covering with anything. That session took a LOT of self-control on my part! All of the massages I have had I was given an option. I have always taken everything off then he would cover me with a sheet or towel. He covers/uncovers half of my butt at a time and massages the part not covered. He keeps my boobs covered (barely) but massages up to the bottom areas of my boobs. He also covers between my legs so that my slit doesn't show. Licensed massages therapists are usually very careful not to go too far with their hands. I did go to the same person for two years (he was gay) and he knew me well enough to do the massage without any drapping.

My level of undress depends on what's being done. If it's my lower back I'm completely nude. No shirt for obvious reasons and pants can interfere when working that low. I kept my pants on at the beginning of my first massage but the way the skin moved around the friction from the waistband became uncomfortable.

If I'm getting my leg worked on for lymphedema I just wear a skirt, take my shoes off, and it's no issue.

It really depends honestly. Usually when I go to an establishment I ask the masseuse what their preference is before I take anything off. Gotta remember, even though you are the customer, it's their establishment, so whatever rules they have in place are the law of the land. Even if I go to a seedy place where I'm definitely looking for a "special session" I still ask "what do I take off?"

The main reason for this is some places will get upset if you take too much off. They may have a rule where you only strip down to your underwear or bottoms. If you don't know, you should definitely ask first. Lastly, if you ask, they will see you as thoughtful and thinking about more than yourself. That can lead to other things IF, and I say IF, they are into that sort of thing.

So, to answer your question, don't strip to anything until you ask.
