I have. And she was relieved , in fact once I was with some friends at a strip club and the dancers tampon string was showing, I quickly pulled her aside to tell her, she turned bright red, ( no pun intended) and ran off stage to fix herself before returning. My concern would be protecting and respecting her dignity. Depending on the setting and whether it seemed like she was unaware about the issue, I might very quietly mention it to another woman so she could tell the woman with the stain about it discreetly. And I would ask the second woman to not mention that I noticed it. But it's important to not overreact to something that may well happen to most women at some point in their lives and to be understanding about it.
