Remember: Japanese アニメ ("anime" from English "animation";「 cartoons」) & 漫画 ("まんが" or "マンガ"; "manga"; 「comics」) are just a medium, like any other from any other country. It all depends on the producer (s). (Interestingly, the Japanese themselves us the Japanese terms for all comics & cartoons regardless of where it was made, while we English-speakers use the Japanese terms for Japanese-made media.)
Generally, Japanese anime/manga are innocent enough for broadcasting across the globe. (For example, my birth-country of the Philippines has been airs the 1970s-era giant-robot anime Voltes V from 1st-episode to last, to 1st again to last, etc. for decades. I would not be surprised if it were still playing even now in 2020. Here is the iconic intro-song almost any Filipino born and raised in the PH will recognize:

Limited by only the imagination of authors & production-crew, as well as their budgets, you can get all sorts of material. Romance? Sure! Enjoy some comedy. Watch some 'slice-of-life' depicting someone's normal, everyday life. Maybe, try an accurate-as-possible historical-recreation, a loosely-based historical-fiction, serious what-if's in history, or even very unrealistic what-ifs. It can be sight & simple or heavy with life-lessons clashing philosophies. It can be for kids (obviously not porn), for teens (porn or not), for adults (porn or not), for the general public regardless of demographics (obviously not porn).
Unless you're in a porn website, it should be safe to assume what's discussed isn't porn. Pornographic material should have tags like 変態 ("へんたい"; "hentai"「perversion」) or terms related to it. Like with American-cartoons, Chinese martial-arts movies, Korean 漫畵/만화 ("manhwa" the Korea's counterpart to Japan's manga), etc. you have to be searching for porn to get it.

Do you need to be fluent in Cantonese or Mandarin to enjoy a Jackie Chan movie? No, right? Likewise. Subs & dubs exist, so understanding the language (at any capacity) is not necessary. Of course, learning some elements of their language and/or culture will help you better understand the characters, circumstances, word-plays, etc. But good translators usually add foot-notes or explanations somewhere to help the audience understand the source-material.
Otherwise (like all else), you learn by exposure. You will learn as you explore.
Anime is just an animated series made in Japan, nothing R-rated about it. It airs on local Japanese tv channels, so it has to be child friendly to a certain extent. It has different genres like scifi, comedy, etc. One you might not know is called ecchi, which is something like "provocative". Meaning big boobs, panty flashes, etc, but no intercourse. This is the kind of show you would see around 9 or 10pm.
Hentai is the R-rated version with actual nudity and intercourse. You generally don't see this on TV except maybe at 3am. (Yuri and Yaoi are specific kinds of hentai, meaning lesbian and gay respectively).
Anime in general is fairly innocent and is extremely mainstream in Japan, it's not uncommon to see big billboard ads featuring anime.
Most of the series that make it over to the west are aimed towards guys 15-30, so you can expect genres such as ecchi, action, adventure, comedy and such more than others.
As some fun trivia, most of the animators are usually female.

I worked on something anime related for a couple of years, so I got some insight :)
Is hard to say. Anime stands for nothing more than being japanese animation. Otherwise, it's just like any western movies or series, with various genres and different anudiences. However, I would say that most likely most of them are at least PG-13, because of blood, cursing and suggestive themes. However, like I said there are kids and adults ones, it's important to check the tags. For example, when something is tagged ecchi, be ready for soft porn, things like panty shots and sex jokes. Hentai, it's the full on explicit in a porn way while eroge is the mature one where sex might indeed appear but it's not in a porn way, more romantacized/hollywood type of sex scene (as in actually relevant to the story).

Anime has many different genres and ratings. Most anime is TV-14 and TV-MA, meaning it's more for teens and adult audiences. When you get into manga it's quite similar, the ratings are usually: +13, +16, +18, and Adult. When you get into anime games those are very much similar to how they rate the manga. However, there is a lot of anime-like games as well so those may or may not come from japan. I'm not saying there isn't any G rated and PG anime, manga, and anime/anime-like games (because there is) it's just that the majority is targeted for teens and adults. If the reason why, it is because it can be pretty sexual and overexaggerates a lot in nature.

Actually I really didn't know that there was sexual porn anime until a few years back.
I remember watching sailor Moon and Inuyasha. Inuyasha is definitely more adult rated.
I also watch Disney miraculous which was supposed to be an anime but they changed their mind and now it's more like a French cartoon.

Me too, I have watched very little of it because Japanese cartoons don't really interest me that much but I am curious. You hundreds, if not thousands, of different types and categories of anime; it's more a form of media than a category. That said, Japanese standards of objectionability are rather different from ours. I think they figure that if everyone can watch sex and violence on TV, no one will go out and commit crimes.

A lot of anime is centered around fan service (sometimes entire series other times single episodes) and it's typically those parts that get a lot of focus. So every anime will be a case by case basis but more than likely people will be talking about the PG to R range of things. XXX anime is called hentai and will always be XXX (every hentai is an anime but not every anime is a hentai).

Anime has multiple genres just like movies or other TV shows. Hentai is basically anime porn. Action anime may contain some perverted scenes but is not the main focus of the anime. For example, History's Strongest Disciple Kenichi is an anime about a high school boy meeting a girl who basically saves him by using martial arts. He learns martial arts to protect what is dear to him, yet he falls in love with the girl and there are perverted moments related to that.

Anime is to broad a spectrum. We'd have to break it down by genres. I dont know the statistics behind which genres are most popular. The ones I watch tend to be R rated but most for gore rather than sexual stuff. The exception to this rule was Berserk's 90's Golden Arc Anime which fucking FANTASTIC! The new shit sucks though. :/

Anime, or animation in general, is not a genre. There's a lot of different shows for different age groups, and there's very different people watching for different reasons.
Most discussion revolve around the most popular stuff which is usually shounen shows and similar aimed primarily at teens and young adults or if you're outside of an anime centered website Ghibli films which are great for any age.
When people talk about NSFW anime they always specify ecchi or hentai.
