I can think of two basis for this insult:

1. It's about homophobia, i. e. to call someone a homosexual is an insult, and there fore calling a male a "cocksucker" is equivalent to calling them a homosexual. There are many more insults/put downs like this, such as "fag", "that's gay", etc.

2. It's about misogyny/degredation of women/women lower than men, i. e. to call someone a woman directly or indirectly, or to say they have feminine qualities is an insult. Therefore, the activity of "cocksucking", whether it is from a woman or a man (see above notes on homophobia), is a denigrating activity that demeans the "cocksucker". Calling a woman this as an insult is saying that women are beneath men, and furthermore, objects for sexual gratification undeserving equal respect.
